Amy’s team works alongside Paramedics, GPs, Care Homes and District Nurses to prevent hospital admissions if this is in line with the person’s current goals for their care. This is particularly targeted at those living with frailty, multiple comorbidities, caring responsibilities or as part of end-of-life care.  

The home visits use bedside testing and a multi-disciplinary approach to provide management of many acute medical presentations with hospital-level treatments. Holistic management plans are developed with the patient, their family, medical teams and care staff.  This way of working requires lots of team working, mutual support, patience and creativity; becoming a member of the UK Hospital at Home Society has encouraged the team to keep developing and exploring.
